Where has Nader gotten on the ballot for November?

Where has Nader gotten on the ballot for November?
I just went through a number of Google searches and have not turned up a single state where Nader is actually on the ballot. Of course, if he does not get on the ballot, he isn't a factor and it is entirely possible that he will not get on the ballot in all 50 states or even a majority.

In Oregon, for example, he needed but 1,000 signatures and could only come up with 800.

Does anyone have info on this?
